Workers Death leads to Charge for Company Director

A firm and its directors face charges for a workplace fatality in Yatala in November 2015, which include a possible 5 year imprisonment or fine of $600,000 and fines of up to $3million for the firm.

It is believed that the victim was using a heat gun close to where a tanker containing water and unleaded petrol was being unloaded when the man became engulfed in flames. He died as a result.

Upon investigation, it was discovered that there was a lack of a hot work permit system, inadequate worker training and insufficient isolation of hot work from flammables.
